Op note reads:
Vertical incision was performed. Dissection was carried down to hernia defect, which was then noted to be approx 1cm in diameter. Just above this was a second hernia defect. There was bridge of normal tissue, approx 1cm in length, between these 2 hernia defects for a total margin of just under 2cm. Both hernia sacs excised. The fascial edges were delineated nicely. A small Ventralex mesh was secured and wound closed with 3-0 Vicryl running suture, followed by 4-0 Monocryl subcuticular closure, and Indermil for the skin.


This sounds like one incision made and should only bill 49560 once rather than twice?

Appears just once would be appropriate, but modifier 22 appended might be appropriate as multiple sacs are not normally encountered.
