20611 or 20610 - Ultrasound image not saved

Hello Guru's,

HOPD Facility coding- The physician documented a knee joint injection with ultrasound guidance but, the images didn't save.
1. Can we bill 20610?
2. Should we bill 20611 with a reduced service modifier?
3. Should the physician add an addendum to the chart stating what happened with images?

Please help I can't find any documentation on this type of situation in a hospital setting.
Please send guidelines if you have them.

I'll keep looking but, hoping you coding superstars have something readily available for this.
