Wiki 5010 billing req and care plan oversight

Our claims for Care Plan Oversight are not going thru on our 5010 billing, does anyone know if we are suppose to put the NPI number of the agency that is seeing the patient? We are putting their 6 digit facility ID and the claim is not going thru, any ideas? kc
