Wiki 70496 and 70498

I am recently getting denials from Medicare HMOs for billing 70496 and 70498 together saying that 70496 is incidental to the primary CPT. Can these not be billed together or does one need a modifier? We have always billed these separately and been paid for both as one is the head and one is the neck. Any help would be greatly appreciated.

How can I see the reply to this question? I'm seeing the same denial and would appreciate any help as well.

In my opinion 70496 and 70498 may be billed together, but You must append modifier -59 or -XS to 70496 if both have been ordered due to medical necessity.
