I am having a major issue with Medicare on this test. This test is in a General Chem 13 panel. We are only certified to do CLIA waived testing, this test is not listed on Medicare's/NGS website as being CLIA waived, however, my rep from Abaxis sent me info from the FDA showing that this test has been approved for CLIA waived status. I have spoken with a Tier 2 person @ Medicare who viewed the information with me and claims I need to refile the claim without a QW modifier, get the denial, and file an appeal and send in the documenatation I have, however, she doesn't think i'll get anywhere since I've got 2 govt. entities saying 2 diff. things (FDA says yes, CMS says no). I've contacted the Ind. St. Dept of Health who recommends I call my Medicare Fiscal Intermediary, whom I cannot locate a number for.
Is anyone else having this problem? Does anyone know where I can find the fiscal intermediary number?
