Can someone advise is if this is sufficient to code 93640-26 or 93641-26 in addition to codes 33249-Q0, 33225, 93641-26, 33241, 99152 and if the other codes are correct?

Pre-op dx: Chronic systolic heart failure non ischemic cardiomyopathy chronic VVI packing ejections fraction less than 30% QRS duration greater than 140 millisecond

Patient is brought to the EP lab where the upper left chest are is prepped draped in the usual sterile fashion. The skin is anesthetized 1% xylocaine. The old ICD pocket in size incision carried down the generator which mobilized free the subcutaneous tissue access gained to the left subclavian vein x2 in using tear away sheath an atrial lead positioned in right atrium and coronary sinus cannulated using venography left ventricular lead placed at a left latera wall vein. Thresholds measured found to be acceptable leads were connected to the Saint Jude Abbot biventricular ICD. Deep tissues checked for non is closed with 3 0 Vicryl skin closed with 4 0 Monocryl running subcuticular fashion. Patient returned to his room.
