Can you bill a 99211 with an RN or LPN as the rendering provider? Is the supervising provider listed on the claim at all? Does the RN/LPN have to be credentialed with the payer?
 
An RN or LPN can never be the rendering provider as they have no NPI number. You bill the referring provider in field 17, this is the provider in your practice that saw the pTient for the same reason they are here today and has the plan of care written in the chart. The rendering/supervising provider in field 24J is the NPI of the provider in the office while the patient is being seen by the RN/LPN.
 
Oklahoma Medicaid replied that they do contract the public health nurses and they want them as the rendering on the claim if they performed the services.
