My hospitalists frequently see patients for admit and discharge say on the 11th even though the hospital admit date was the night of the 10th. I have been billing these as 99234-99236 when both services are provided on the same day. Do codes 99234-99236 require separate face-to-face visits being documented for the H&P and the discharge in order to appropriately report the services? I have ran into two patients, where the physician documented only the H&P and within that stated patient will be discharged home.
