Appropriate 59 modifier usage with EKG

Is it appropriate to add a 59 modifier to 93005 when the patient has a surgical procedure and no diagnosis appears for the EKG on the LCD. This is for a hospital technical charge.

If the reason is ""preop"", the -59 modifier can be added.

If you can determine medical necessity (chest pain, atrial fib, etc.) you can also add the -59 modifier.

If the EKG was part of a procedure, for example, monitoring during a procedure, or if the EKG was merely to check the result of the procedure, you must write-off the EKG."
