Wiki Are any of you submitting the maximum 12 diagnosis codes per outpatient visit? If so, how?

We are having trouble submitting more than 4 ICD-10-CM codes on our outpatient visits. Our clearing house says that they are submitting up to 12 but only 4 are being accepted by CMS. Does anyone have any suggestions?
