"Wanted to know if it's possible to submit two separate visits for the same patient by the same physician on the same day. The patient was seen in the am left and return a few hours later for a different medical problem"
If the pt is seen for different reasons, different dx at each visit, I would bill both E&M's with -25 modifier. If they were seen for the same reason, I would add time, bullets, etc,. together to get total E&M level with Prolonged service, if applicable.
