Wiki billing/coding for in office surgery suite

For those who have an in office surgery suite (use conscious sedation or general anesthesia), how do you bill/code to insurance companies. Do you bill 1 line item using the procedure CPT and then have the price include the "room fee" or do you split it out into 2 items (1 for physician, 1 for "room fee", how would you code that if you do?)? We are not a surgery center so we aren't quite sure the best way to do the billing.
