Wiki Billing of supervision of patient under home health care CPT code 99347

Hi, I have been researching the billing of 99347 for the care provided for a patient while they were under a home health agency. The patient was admitted in and out of inpatient hospital status during the 4 month period. There is a Home Health detailed episode report that was updated per month with documented location of the power of attorney at patient's home, for a patient while in patient at the hospital. My question is, when billing the POS, is it the inpatient 21 while the patient was in hospital or patient home 12 for power of attorney? Or is there more to it? Is anyone familiar with the billing of this code?

Just reaching back out, if anyone is familiar with billing the POS and location on 99347? I also found in the category for 99347 in the CPT book, it states to refer to 99374 if the patient is under a home health agency, so feedback on the billing of that is also requested.
