Starting in October, Colorado Medicaid is not paying for a lab handling fee with an E/M. I cannot find anything about this in the Coding guideline or in their Billing Manuals and wonder why the change?? A Modifier 25 does not work even though they suggested I try that. Does anyone know where I can find specifics about code bundling with E/M and Counseling codes?
 
Have you found a solution or new coding guideline? I'm having the same problem with Medicaid not paying for Critical Care on the same day of a procedure, even with modifiers 25 and 57. They will only pay if the patient is seen by 2 different physicians. We're working on appeals.
