Child vs Adult vaccine administration

Okay Im confused. When I have a child under the age of 18 come in for their immunizations ie Tdap and counseling is documented by the provider I will enter 90715 90460, 90461x2 but going by the July issue if the provider does not document that he counseled the patient and/or parent I can't use 90460-90461. So would I than enter 90715/90471 only? Also if I give an adult this vaccine would I code it as 90715 90471,90472,90472 or 90715 90471??

Okay I have to examples and tell me what one is right. I want to make sure I'm coding the administration of imms correctly.

Okay for example we will go will Tdap its a vaccine with mulit components in one shot that can be given to a child or adult.

Example 1
Child is seen by nurse only for injection

Example 2
Est Adult patient is seen by Dr for chronic issue and updates his tdap.

You would use 90471 and 90715 for the child and the adult. No nurse visit for immunizations. If the child has state insurance you would only bill for the 90715. If you have state supplied vaccine, you would bill commercial insurances the adm. code only. It's my understanding that you wouldn't bill each component if the nurse gives the vaccines, only if the provider counsels the patient and he/she has to document that they discussed each component. It's a lot of documentation.
