There are 4 types of CLIA Certificates.

1. A certificate of waiver allows a laboratory to perform only tests categorized as waived.

2. A certificate of PPM allows a laboratory to perform tests categorized as provider perform microscopy procedures and, if desired, waived tests.

3. A certificate of compliance allows a laboratory to perform tests categorized as waived, PPM, moderate or high complexity.

4. A certificate of accreditation allows a laboratory to perform tests categorized as waived, PPM, moderate or high complexity.

When a lab with certificate of waiver performs a waived test they are required to use modifier QW.

When a lab has a ceritificate of PPM they are allowed to perform waived test, but CAN THEY BILL THE WAIVED TSET WITHOUT MOD QW?
 
Medicare denying with QW

We performa few waived tests in the office. We are a waived lab under CLIA. Medicare is denying these charges with remark notes of "CON, GRA, R1" What am I missing?
