Good Morning - would like opinions on the following closures - Simple so can't bill closure; intermediate; or complex -

was billed with 11402: the lesion in its entirety was removed. This will then be sent to pathology. Hemostasis 1 Lesion 1 was obtained. 1 consistent with sebaceous cyst and discarded. 1 The wound was irrigated and closed with 3 0 Vicryl sutures followed by nylon 1 Prolene 1 sutures in an interrupted fashion. A dressing was then applied. The patient tolerated the procedure well.


was billed with a 11603- Lesion excised to the underlying fat. there was tension when bringing the wound edges together, therefore circumferential undermining was undertaken. Wound irrigated and closed multiple layers with 3 0 Vicryl followed by 3 0 nylon. Multilayer closure needed to prevent wound dehiscence. Margin required 2.5 centimeters, length 7 centimeters.

was billed with a 11602- Lesion excised to the underlying fat. there is noted to be tension on the skin edges with attempted closure, therefore circumferential undermining was undertaken. Skin edges then came together without tension. Wound irrigated and closed multiple layers with 3 0 Vicryl followed by 3 0 nylon. Multilayer closure needed to prevent wound dehiscence. Margin required 1.2 centimeters, length 7 centimeters.
 
per the CPT book under Complex repair since undermining was done greater than or equal to the max width of defect plus it's layered closure(intermediate) the last too would be complex repairs:)
