I've received a Coding Clinic response as to how to handle provider usage of "with" or "associated with," such as hyperlipidemia associated with diabetes. I work in HCC and insurance companies want the E1169 as the provider has supposedly linked the conditions. Other coders also seem to think this is how it should be. I've received Coding Clinic advice to the contrary. I've seen others post responses that also conflict with how in practice situations are coded, but unless The Coding Clinic publicly publishes the responses, what good are their responses? I wish coding was more black and white, with better guidelines. Has anyone else felt discouraged by the grey areas of coding?
