Wiki Copayment collection spread sheet

The center that I currently work for does not collect copayments from our patients. However, we will be providing services to another facility and I would like them to start this process right from the start. Does anyone out there have a spreadsheet ready to go to document the copay, DOS, amount, patient name, etc.?
