You could be getting denials due to not adding contrast material A9552? Or denied if this a repeat CT to compare if done before? If so add modifier PS or if initial CT of the heart add modifier P1 per results should tell you. Also add proper dx code? Are you using I50 or I25 or I44 or I77 cardiac dx codes not just Z13 .6 codes as first dx code? Also if got pacemaker add that as last dx code Z95 .0 or heart transplant dx Z94 if warranted per pt s past medical history.
