Wiki denial due to inappropriate modifier

This what our claim went out like:

96372
J1885-51
J3420
17110-59

All was paid by Humana except J1885 citing the inappropriate use of the modifier. Should -51 be attached to 96372? Or should 96372 been on the claim twice for each injection?

I do agree with Jennifer if documentation supports the charges. Modifier 51 can be problematic and in your case, not even necessary.
