Opinions needed:

A patient brings in an order for lab exams along with an office encounter summary. The summary contains diagnoses not on the order itself. Would you allow the codes from both the order and the encounter summary to go on the claim?

Example:

Order for PSA, CBC, TSH and urinalysis.

Order contains diagnosis code of 600.90 (enlarged prostate).

Encounter summary contains diagnoses of 600.90, hypertension (401.9), high cholesterol (272.4), anxiety (300.00).

What code(s) would be on the claim for the lab exams?
