My doc did partial amputations (28820 or 28288) of the 1st and 2nd DP toes and a FTSG on each of the 1st and 2nd digits. He said he had to create two separate grafts. Do you bill for 2 separate grafts as 15240 and 15240-59 or do you add the sum of the areas. He insists that he should be paid separately. Can you also bill the 15004 for preparation of the site.
