Wiki Documenting on a different provider note

I have a chiro provider who is doing his documentation on a othro provider's documentation.

The ortho provider submits his documentation for medical management to the chiro, the chiro scribbles his documentation on the ortho providers note. Where can I find guidelines about this situtation?

each physician, in order to submit a claim, must document his own note. From what I understand of your question, do these 2 dr's work in the same office with the same group NPI and tax ID number?
It sounds as if they are simply being lazy. I would be very concerned about this set up and their records being audited.

No these are two different providers working in different scopes of practice the one who is scribbling his notes is a chiro doctor on to the orthopedic's documentation. I believe the ortho is submitting his documentation to the chiro because he see them for ortho problems and is able to give perscriptions.
