If you bill an E/M visit with a prolonged code, do you add the -25 modifier to the E/M? For example if I was going to bill 99213 and 99354, should the 25 modifier be on the 99213?
Please help.
Modifier 25 is utilized when an E/M service is performed at the same date by the same provider with a sevice coded from CPT range 10000-60000. In this scenario you don't append modifier 25 to another E/M code when solely performed with another E/M code. If you were coding a E/M code 99213 and a code from range 10000-60000, then it would be applicable to append this modifier to the E/M code.
