Can we code 99211 with procedures 36591 (venipuncture from indwelling port) and 36415 (venipuncture)? Is this double dipping?

If so, do we need to use modifier -25?
 
No you may not use a 99211 when the purpose of the visit is to draw blood, the nurses work and supervision is already included in the blood draw codes.

Do I code 99211, or the 36591 & 36415? Or a different code(s).
 
If the patient is comming in for a blood draw encounter as instructed then you use those codes and not the 99211. You may use the 99211 for an encountered that has been ordered by the physician, yet qualified ancillary staff (nurse) is the one with the patient while the physician is within the office and there is no other CPT code for the activity performed.
