Does anyone do earpiercing in your office and if so do you have the parents sign a waiver? We are looking into this service and I was wondering how other offices handle it.

I work in a Pediatric office and we do ear piercings often. We do not have the patient sign a waiver specifically for this because we do the piercings as Self Pay. Patient has to pay upfront for this service in order to have it done. We do not bill health insurance for this. We do have patients sign a waiver yearly for any service that may not be covered.Hope that helps. :)
 
Self-pay cosmetic procedures

This would be considered a self-pay cosmetic procedure. Payment in full, in advance at least 10 days prior to scheduled procedure. We don't bill these to insurance at all.
