Wiki Easy way to know which E/M code to assign?

I'm a new CPC-A coder and I'm working on my Practicode. Whenever I have to code an E/M service, I often get it wrong. I know this is pathetic but is there an easy way understand which code to use? For example, I know that an Emergency Department visit is codes 99281-99285 but I often choose the wrong code within that range. I think some kind of chart would be super helpful.

I googled "E & M and Evaluation and Management worksheet" or "E & M audit worksheet" and found 5 different forms, one from CMS, another from AAPC. They were all very helpful.
