I need advice to what to code for exchange of G-J tube to G-tube. Should the G-j tube removal be coded and then the placement of G-tube? Any suggestion will be helpful?


PROCEDURE NOTE
Diagnosis: Gastrointestinal dysmotility
Procedure: Exchange of G-J to a G
Findings: New gastrostomy tube inserted without difficulty.
Indication: Patient has severe gastrointestinal dysmotility with gastroparesis which requires continuous drainage of the stomach.  This had been accomplished with the gastric portion of a G-J tube, these tubes frequently required repositioning, so the patient had a Roux-en-Y jejunostomy tube placed.  This works well, so exchange to a G-tube only is warranted.  When this was attempted by a home health care nurse, it was felt to be under undue tension and they aborted the attempt.
Description: The balloon of the G-J tube was deflated.  It was withdrawn with only a small amount of tension needed to get the deflated balloon out.  The tube was removed in its entirety.  The new tube was positioned and the balloon was inflated.  Gastric contents were withdrawn.  The tube was placed to straight drain.
