Wiki Family HX of Colon Cancer

I know Medicare does not consider a grandparent with a HX of colon cancer as a close enough relation to code V16.0, but do all carriers follow the same guidelines on this? This DX/Indication gets used alot by doctors and often times the relative falls out of the group Medicare will allow.

I find that most carriers have their own policy regarding colonoscopies. You can find them on each carrier's website. I also find that most of them regard family hx as first degree relative only or at least two second degree relatives.
