If the PCP sees a patient for ongoing issues that are managed by other specialists, how does that affect the MDM? The PCP takes the history from the pt of the care he's been getting elsewhere and the pt tells the PCP he has an appointment to follow up with the specialist. In the assessment/plan, the PCP notes "Follow up w/Dr.____, as planned". A provider is asking for a 99214 for one patient who has 3 chronics on medication, but who is being followed elsewhere for all 3 chronics. Do issues that are totally being managed elsewhre count towards the MDM? I'm having a hard time finding the medical necessity for the visit itself, other than updating the pt's chart. Thanks for any advice!
