Our Internal docs do a foot exam when a patient has diabetes, to code I do an
E/M code with a 25 mod. and add the G0245. I have tried to do a 59
and Medicare will not pay. Does any one know what I need to do to get this
paid we have the documentation and everything

as it looks like you posted this a few months ago. I am wondering myself. It appears that the NCCI is saying you can not bill a E/M with the code but I am not a expert on NCCI.
