Wiki Health Risk Assesment for non verbal Patients

We are having an issue. Our House call Providers perform a Health Risk Assessment at the time of the annual exams. Problem is, some of our patients have dementia and cannot communicate verbally. Does anyone know the requirements for this part of the annual exam, if at the time of the visit there are no family members present and the majority of the questions are self explanatory. For example "are you able to bath yourself? Or are you able to go food shopping by yourself?

We cant find anything that specifies if we can or Can not bill for the annual exam without this component. Or can we bill for the annual exam as long as we document why this component is not done?
