patinet came in for IUD insertion also asked the physician address abscess that was I&D in er a few days ago. Due to failure to insert IUD patient was given DEPO shot.

How would this be coded?

Wound care note:
Aerobic and anerobic cultures were obtain from the incision. The incision was then cleaned with saline and wet to dry packing with sterile gauze was applied and incision covered with 4x4 gauze.



IUD Procedure Note:
The cervix was prepped with iodine swabs times 3. The anterior portion of the cervix was grasped with a single toothed tenaculum. Gentle traction was used to straighten the cervical canal. The uterine sound was advanced to 4cm and resistance was noted. After several attempts and not being able to go through the internal cervical os, an endometrial pipette was used to sound the uterus becuase of it's flexibility and to decrease the risk of uterine perforation. The endometrial biopsy was unable to be advanced past the internal cervical os. Therefore the procedure was aborted. The single toothed tenaculum was removed and pressure applied to the tenaculum sites and hemostasis was noted.

I would add modifier 52 to the 58300 as modifier 53 is more used for when a procedure is discontinued due to extenuating circumstances or those that threaten the well being of the patient.
When our doctors cannot insert an IUD we bill it with the modifier 52 not 53.

Also code 90782 is not a valid code 96372 is the administration of a drug.
