I am coding for a new ortho doc and when he sees a patient for a fracture he is charging for fracture care and cast supplies, not the application, which is correct; however, at every follow up he is removing the initial cast to do an x-ray and then applying a new cast after the x-ray. Can he charge for a cast application and supplies at each follow-up if I append a modifier 58?

You can't charge for the initial cast, but any casts after that you can charge for. . . . .

It is odd that he would 'need' to do that but if he documents it you certainly can bill for it.
