You could use it for either element but I would probably put it towards severity.
Severity is best when it is rated 1-10 but you don't always get that in a note. Any word that gives a picture of how bad it is, (ie minimal, significant, extreme) I tend to apply to severity

Significant=Reasonably large in number or amount, Having a noticeable or major effect.
I would credit towards severity, also.
