Humana PFFS & Oxygen Contents Billing

I work for a local DME office and we have noticed that Humana has been denying our oxygen contents claims stating that the equipment is purchased. We did not have any problems until the last month or two. We have been calling Humana and they, unfortunately, are not of any help and keep telling us that if we don't like their determination then we can file an appeal. I'm wondering if anyone knows of a policy change that we didn't catch on to or if you are having similar issues how are you getting Humana to correctly process the claims for contents (once the equipment is capped). Thanks!
