The procedure the physician performed was an instillation of lidocaine into the sinus cavity using a SphenoCath (not an injection). The physician report states the procedure was done under "lateral fluoroscopic guidance" and wants to bill 76080 in conjunction with the procedure code. I have found conflicting sites with examples for this code, some saying 76080 can only be used when there is an abscess or fistula in the sinus cavity and others that support the use of this code for imaging of the sinus cavity alone. Is 76080 appropriate for fluoroscopic guidance during SphenoCath sphenopalatine ganglia block (topical instillation)?
 
The code descriptor for 76080 is used for the imaging of an abscess, fistula or sinus tract. It is used with codes 20501 injection of sinus tract or 49424 injection of abscess or cyst via previously placed drainage cath. It would not be used for imaging of the nasal sinus.

When a sphenoganglion block is performed using a device to deliver medication through the nose, report code 64999. Do not report code 64505.
