Wiki is it a MUST that the physician document the exam?

Hi,

Can anyone verify who MUST document the exam?
Is it true that the physician must document everything but he/she can use the nurses note for the vitals?
I can't find it anywhere in 95 or 97 DG
If anyone has it in black & white can you please send me the link?
