Coding Guidelines
Factors influencing health status and contact with health services (Z00-Z99)
Note: Z codes represent reasons for encounters. A corresponding procedure code must accompany a Z code if a procedure is performed. Categories Z00-Z99 are provided for occasions when circumstances other than a disease, injury or external cause classifiable to categories A00-Y89 are recorded as 'diagnoses' or 'problems'. This can arise in two main ways:
(a) When a person who may or may not be sick encounters the health services for some specific purpose, such as to receive limited care or service for a current condition, to donate an organ or tissue, to receive prophylactic vaccination (immunization), or to discuss a problem which is in itself not a disease or injury.
(b) When some circumstance or problem is present which influences the person's health status but is not in itself a current illness or injury.

Hello-
We are having a difficult time with coding pap smears in the lab/specimen setting. We have a pap smear we want to code for billing that needs the dx code for pap (Z12.4) and they also got it screened for HPV (Z11.51) but the Z12.4 has an excludes 1 note and cannot be coded with Z11.51. So how do we code the pap smears that had HPV screening as well?

Thank you for the help.

I would really like an answer to this as well

Is it okay now to code them together? What if the order Dx only states Z00.00 and a Pap and HPV test were done. Can we code them together now? I hope someone who knows can answer this. I'm coding facility outpatient Lab/Rad/Path.

We had the same problem and were told by a large national lab to use Z01.411 or Z01.419 for cervical paps. Insurances are paying with these diagnosis codes and are paying for the HPV screening (Z11.51).
