Wiki Lab cpt/icd-9 coverage crosswalk

I have been reviewing the Medicare website trying to find a listing per cpt code (in particular labs) with the covered diagnoses for that lab. Does anyone know where this is, b/c I can't find it. Maybe I'm crazy and it was never there.....but, I really think it is!
