We have billed 87426 out to several Medicaid products electronically and our QW # goes over on the claim. We have only billed a few Louisiana Healthcare connections and have received denials stating we need to add the correct modifier for our state? We can find no information and the Medicaid clear claims connection does not knock it out needing the QW modifier. Has anyone else experienced it?
