Patient DX: Lumbar HNP & and lumbar spinal stenosis

Physician performed lumbar laminoplasty-style laminectomy - at L4-5 and L5-1 left sided laminotomy with ligamentum flavum removal. At L3-4 left sided leminotomy and meical facetectomy with ligamentum flavum removal from right side after undercutting right l3-4 facet joint.
