Wiki Managed Conditions

Good Morning,



I have a question regarding counting conditions as managed. If the condition is not mentioned anywhere in the note other than in the assessment and plan and links a med refill or lab to the condition and there is evidence of a face to face visit would this condition be considered managed at this encounter?
