If patient is a minor can they be seen by themselves without an adult bringing them?
If no, do they have to be brought in by a relatives only?
Is it acceptable to have a verbal consent over the phone or written is a must? What form is a consent should be in?
Can you point me to a reliable legal reference on the matter.
 
This is something that is governed by state law, so the rules will vary depending on where your practice is located and you'll need to check your specific state's regulations for a definitive answer. Generally speaking, consent of the parent or guardian is required to authorize medical treatment for minors in most cases unless the minor is emancipated but there may be some specific exceptions for certain kinds of treatment. I can't advise on whether or not a verbal or phone authorization can be accepted or whether relatives can authorize treatment but it would seem to me to be a risky practice.
