Wiki Modifier 52 for Liner Exchange

I have a question regarding modifier 52. Is it true that there is an automatic 50% reduction in fee services? We have cases were a physician will do a complicated liner exchange on a knee revision where probably 80% of the work was done but the insurance will see modifier 52 and do an automatic 50% reduction. If the liner exchange is complicated, can we avoid amending the modifier 52 or does it still have to be reported
