Hi! I'm receiving conflicting information when documenting whether to use the NDC# on the packaging or the vial/injection for a claim? Does CMS actually have guidelines on how this should be documented?

Hello, There may be something on the NDC crosswalk lookup on the CMS website that specifies this, but I didn’t have a chance to review that.

We always use the NDC that’s listed on the actual vial of the medication. I only have one payor, might be UHC that gives us trouble occasionally, and won’t recognize the vial NDC and wants the package NDC. I have never had an issue with MCAS though.
