We have a patient that had a sling placed last November (90 day global); the vaginal mucosa separated and she was taken back into the operating room by the same physician on 1/11/12 to have this reapproximated. I did not find any code that I felt described this procedure so I am leaning towards billing 58999 with mod 78 and diagnosis 998.31. Any thoughts/comments?
