My billing boss asked me a question as the only coder on staff; since I am new-ish I wanted another opinion: New patient (pediatrics) has a well visit and other things come up so they also have an 'office visit' at the same time. The question was new/new or new/established. I said new/new as date of service is the same, as it's all the same visit.. Is this correct?

New well, establish OV. But make sure there are enough issues that warrant an OV. Minor things that come up are included in the well exam.
