Can a physician bill for a non-diagnostic ultrasound in an office setting for purposes of clarification of swelling and pain under the clavicle? Ultrasound showed mild inflammation in the muscle fibers. Does he need a "picture" of this in order to charge for the ultrasound?

Thanks.
